Preparing for your appointment with us
If possible, please print and fill in the necessary forms before your first visit. You can bring to your first appointment, email or fax to 250-598-3317. If need be, you are welcome to come in about 20 minutes before your appointment to fill in the forms in our office.
If you have recent blood work, scans or tests done by your MD or other health providers, please bring with you. If you do not have recent tests results we can send for those by having you complete the Copy of Medical Records form. If possible, please print and fill out a copy for each pertinent health care provider and bring to your appointment, email or fax to 250-598-3317. We will take care of the rest. This aids us in a having a complete picture of your health.
